Lock Broker Registration
Disclaimer: After submitting the following information, you will receive an email to complete registration. Note that you must have at least one active sold account with the health plan for a successful registration. If you need assistance, please email shpbroker@sutterhealth.org.

Broker ID and Agent ID are Mandatory. Enter each field in the required format and click the "Add" button, to include the Broker Account in list below, before submitting the request. If registering for multiple Broker Accounts, you will need repeat these steps for each Account you wish to access.
